Perceived

Lead in a child's spice or fruit pouch is a fear that did not exist in most parents' mental models before late 2023, then arrived suddenly as a national news event. The mental model that does exist tends to attach lead to old paint and old pipes, not to a sealed, brightly packaged organic-marketed fruit puree handed to a toddler as a snack. In the winter of 2023 a counted thing happened that is hard to assign a probability to. Between late October 2023 and April 2024, CDC logged 566 cases of lead poisoning across 44 states, the District of Columbia, and Puerto Rico, tied to cinnamon-flavored apple puree pouches sold under the WanaBana, Schnucks, and Weis labels. Of those, 542 — about 96 percent — were children under six, and 311 were under two. The median peak venous blood lead level among reported cases was 7.2 micrograms per deciliter, roughly double CDC’s 3.5 reference value, with individual children reaching 39.3. Against the roughly 22.4 million US children under six alive during that window, 566 reported cases works out to about 1 in 40,000, and the 130 laboratory-confirmed cases alone to about 1 in 172,000. Both numbers describe a single historical outbreak, not a recurring per-year rate, which is why this entry is scoped to that one event rather than to a lifetime.

The reason a lifetime figure would be dishonest is the mechanism. FDA testing of the cinnamon that Ecuadorian supplier Negasmart sold to the manufacturer Austrofoods returned lead concentrations of 2,270 and 5,110 parts per million — against an international draft standard of 2.5 ppm for lead in bark spices. The recalled finished puree tested at 2.18 ppm, more than 200 times the 0.01 ppm action level FDA has proposed for baby and toddler food. The lead arrived as lead chromate, a bright pigment historically added to spices to inflate their weight and deepen their color, and FDA’s stated leading hypothesis is that this was an act of economically motivated adulteration. That is a supply-chain fraud event, episodic and discrete. The headline also flatters the denominator in a way worth stating plainly. It divides the cases by every US child under six, almost none of whom ever ate the product. The conditional risk for a child who actually consumed a recalled lot was far higher — CDC built its case definition around recent consumption precisely because exposure was the near-necessary condition for an elevated level. The episode is best read as one realized instance of a recurring vulnerability rather than a settled rate: through 2024 and into 2025 FDA continued to issue public-health alerts naming additional ground-cinnamon products at 2 to nearly 8 ppm lead, and the agency notes there is no safe blood lead level in children. The dominant lifetime lead pathway for US children is still deteriorating paint and plumbing, with roughly half a million children carrying an elevated level at any time from those ambient sources.
